What it is:
Alamance ElderCare’s process to actively listen, answer questions, and provide information and referrals to community resources.
Purpose
Provides older adults and family caregivers with information about community resources that promote well-being, the ability to remain safely at home, and independence
Who Can Benefit:
Any member of the community
Outcome
- Staff often identify other supports/programs that could be of assistance
- Assistance to those who need help creating a plan and obtaining services.
- Provides information and referral to resources within the community.
